Premium subsidies: from an entitlement to a reduced invoice

Premium subsidies are an essential part of socially balanced health-insurance funding. Their result cannot be measured by the planned budget alone: eligible people must receive the relief in time to pay their premiums.

Relief becomes effective when an established entitlement reaches the invoice; each stage should be traceable.

A federal framework with cantonal procedures

The Health Insurance Act requires cantons to reduce premiums for people in modest economic circumstances. Within federal requirements, cantons determine eligibility, amounts and procedures. Some grant relief automatically; elsewhere an application is necessary. The relevant information therefore depends on the canton of residence.

Minimum requirements for cantonal financing contributions apply from 2026. They address resources but do not themselves settle how every household obtains help. A funded scheme can still contain administrative barriers. Those dimensions require separate examination.

Where an application can stall

The journey begins before the form: knowing a potential entitlement exists and finding the responsible office. Documents, assessment and a decision follow. Payment goes directly to the insurer, which then reduces the invoiced premium. Different actors are responsible for these stages.

When an invoice fails to reflect expected relief, it should be possible to distinguish incomplete documents, a pending decision and a transfer not yet applied. The PCP supports understandable case-status information and an appropriate contact. This avoids repeated approaches to offices that do not yet hold the file.

Automation can improve access where the necessary data are available and accurate. It must retain ways to report exceptional circumstances and correct information. A fast decision based on old data is not thereby a fair one.

Income can change before the file does

Job loss, reduced employment and family changes can alter disposable resources quickly. Tax information concerns a particular period and may not reflect current ability to pay. The FOPH identifies consideration of the most recent economic and family circumstances, particularly at the insured person’s request.

The PCP supports accessible ways to have those changes examined. Required evidence and the date a decision takes effect should be clear. This does not presume an identical entitlement in every canton; it means making the system usable without requiring people to interpret a regulation alone.

Examine the remaining burden

Subsidies reduce premiums; treatment cost-sharing is a separate expense. The protection provided should be assessed against disposable income and remaining healthcare costs. Beneficiary numbers alone can conceal small awards or changes in the group covered.

The PCP supports solidarity that avoids abrupt losses when income moves just above a threshold. Cantonal choices should be discussed using clearly defined household examples, without converting a simulation into an individual promise.

A sound assessment connects recognised entitlements, completed applications, processing times and amounts actually credited. It cannot assert a non-take-up rate without a method for estimating it. The political purpose is for relief to reach its intended recipients and improve their ability to fund necessary care.
